B. Documentary Requirements
- Photocopy of Grade 12 Report Card (for First Year) or Report Card for 2nd semester 2022-2023 (for Second year and Third Year)
- Photocopy of Live Birth Certificate (NSO/PSA)
- Original Copy of the Certificate of Tax Exemption/Income Tax Return of parents issued by the BIR
- Photocopy of Baptismal Certificate (at least Catholic Religion)
- Original copy of Recommendation Letter from the Parish Priest
- One piece 2×2 ID picture with white background
- Accomplished Grant-in-Aid Scholarship Application Form (USA-ASP-F09)
NOTE: Students from CNND and CPMT are not eligible for the said application.
