C. Documentary Requirements
- Photocopy of Grade 12 Report Card for 1st year & Grade 10 for Grade 11.
- Original Copy of the Certificate of Tax Exemption/ Income Tax Return of parents issued by the BIR
- Certification issued by the school principal indicating the total number of graduates and honors received
- Accomplished Scholarship Application Slip (USA-ASP-F44 & USA-ASP-F45)
