KINDLY OBSERVE THE FOLLOWING TIMELINE, SCHEDULE AND GUIDELINES:
A. Schedule and Timeline
Distribution of Scholarship Renewal Form for ALL COLLEGES
When: September 11-18, 2023
Time: 9:00 AM – 11:30 AM
2:00 PM- 4:30 PM
Saturday is until 12NN only
Where: Admissions, Scholarships and Placement Office (ASPO)
B. Documentary Requirements (FOR STRICT COMPLIANCE)
- Photocopy of Second Semester AY 2022-2023 REPORT CARD WITH COMPLETE GRADES;
- Photocopy of Registration Form (RF) First Semester AY 2023-2024;
- Photocopy of Student Identification Card (ID);
- Duly filled-out Application for Academic Scholarship Program Form (USA-ASP-F10).
C. The Academic Entrance Scholarship’s Program Policies and Guidelines was approved by the Office of the President (OTP) effective FIRST SEMESTER AY 2022-2023
