F. Documentary Requirements
- Photocopy of Latest Report Card for 1st Semester A.Y. 2024-2025.
- Photocopy of Live Birth (NSO/PSA)
- Original Copy of the Certificate of Tax Exemption/ Income Tax Return of parents issued by the BIR
- Photocopy of Baptismal Certificate (at least Catholic Religion)
- Original copy of Recommendation Letter from the Parish Priest.
- One piece 2×2 ID picture with white background
- Accomplished Grant-in-Aid Scholarship Application Form (USA-ASP-F09)
G. Qualifications
- First year college from CLASE, COC & COT.
- Shall pass the interview to be conducted by Admissions, Scholarships and Placement Office (ASPO).
- With average grade of at least 83% during her/ his Grade 12.
NOTE:
- Students for CNND & CPMT are not eligible for the said scholarship program application.
- Incomplete requirements will not be processed.
