To all Second Year, Third Year, Fourth Year and Fifth Year regular CHED scholars (Half and Full Merit grantees), kindly fill out the Google Form: https://docs.google.com/…/1NfJlxRHyxstMOPGJfMiL…/edit… for the renewal of your scholarships for 1st Semester, AY 2020-2021, starting October 09, 2020 until October 20, 2020.
Note:
1. Must upload the report card with the signature of the College Dean.
2. In the absence of the Registration Form (RF), please submit Certificate of Enrollment issued by the Office of the Registrar.
3. Encode your USA email account.
