  1. Report Card for College (Latest grading period) or Transcript of Records (Graduate Studies only)
  2. Certificate of Honorable Dismissal (Graduate Studies only)
  3. PSA Live Birth
  4. Good Moral Certificate
  5. Admission Fee of 1,000 and Downpayment of 1,500 must be paid separately. Proof of payment must be sent
