B. Documentary Requirements
1. Photocopy of Grade 12 Report Card
2. Photocopy of Certificate of Good Moral Character
3. Photocopy of Live Birth (NSO/PSA)
4. Photocopy of Admission Test Result
5. Original Copy of the Certificate of Tax Exemption/Income Tax Return of parents issued by the BIR
6. Photocopy of the Baptismal Certificate (at least Catholic Religion)
7. Original copy of Recommendation Letter from the Parish Priest
8. One piece 2×2 ID picture with white background
9. Accomplished Grant-in-Aid Scholarship Application Form (USA-ASP-F09)
C. Interview will be scheduled and will be communicated through email or text message starting August 14-16, 2023.
