A. Schedule and Timeline of Activities

Distribution of Application Form
September 20, 2023 – September 30, 2023
Time: 8:30AM – 11:30AM and 2:30PM – 4:30PM
Venue: ASPO

Submission of Application and Complete Requirements
October 02, 2023 – October 14, 2023
Time: 8:30AM – 11:30AM and 2:30PM – 4:30PM
Venue: ASPO

B. Documentary Requirements

  1. Photocopy of Grade 12 Report Card (for First Year) or Report Card for 2nd semester 2022-2023 (for Second year and Third Year)
  2. Photocopy of Live Birth Certificate (NSO/PSA)
  3. Original Copy of the Certificate of Tax Exemption/Income Tax Return of parents issued by the BIR
  4. Photocopy of Baptismal Certificate (at least Catholic Religion)
  5. Original copy of Recommendation Letter from the Parish Priest
  6. One piece 2×2 ID picture with white background
  7. Accomplished Grant-in-Aid Scholarship Application Form (USA-ASP-F09)

NOTE: Students from CNND and CPMT are not eligible for the said application.
