B. Qualifications
1. Enrolled BS Pharmacy student
2. With General Weighted Average (GWA) of 85% during Senior High School
C. Documentary Requirements
1. Letter of Intent Addressed to the President of the Rotary Club of Metro Iloilo
2. Photocopy of Report Card with latest grading period or with complete grades
3. Original and Photocopy of Parent’s BIR/Income Tax Return
4. PSA/NSO Live Birth Certificate (photocopy)
5. Original copy of Certificate of Indigents issued by the Brgy. Captain
Note: There will be an initial and final screening to be determined by the ASPO.
